10/25/2023 EnchantaVaughn Approved Behavior: Upon arrival to the home setting, QMHP engaged mother in a discussion about client's behavior since the last session. Mother verbalized to QMHP on how client has been arguing with her peers in the community then bringing the attitude back into the home and arguing with her grandparents. Behavior: During the second portion of the session, client needed some redirection from QMHP as she was getting angry with her younger sibling for coming into her session. Intervention: QMHP coached client to acknowledge and list her strengths, resilience, and the positive outcomes that often occur when she makes informed decisions opposed to impulsive ones. QMHP verbally identified the client's impulsiveness and encouraged the client to use self-control strategies (i.e. decrease hyperactive behaviors. QMHP reviewed several strategies for conflict resolution and positive social behaviors with client in efforts to increase client’s awareness. The client was encouraged to use good judgement, and respect authority figures. Intervention: QMHP provided simple praises to client for her commitment to her progress. QMHP praised and recognized client for her hard work, efforts, participation, and working towards improvement by changing daily habits and implementing a positive and consistent routine. QMHP supported client with meeting her objectives and coping with stressors without needing additional redirection or prompts. QMHP complimented client for utilizing acquired skills, paying attention, and focusing on the importance of the assignment. Response: Client was able to speak calmly and demonstrate her improved verbalization skills while conversing with QMHP. Client thanked the QMHP. Client was acceptive of the interventions provided by QMHP. Client understood the importance of listening and following directions and have reduced time spent outside to learn more about her wrongdoing and shows initiative to correct them. Client utilized relaxation strategies. Client was seen decreasing anger and frustration. Client reported a decrease in symptoms following the relaxation techniques. Response: Client verbalized positive feelings about achieving goals and objectives. Client communicated that she wants to work independently. Client displayed good participation in various settings and was responsive to all instructions, interacting well with her peers. Client utilized coping skills and impulses started decreasing. Client was observed strategizing a plan to better himself by writing down her thoughts and feelings and exploring her creative side with arts and crafts. Client was taught to take deep breaths and count to 10. Client was reported displaying age appropriate behaviors and was productive and studious in the home today.
